TERMS OF AGREEMENT *

I understand that I will have to pay prior to services. I understand I have to complete services within 6 month time frame. No extra services will be performed after 6 month term. I understand there will be no refund if I do not complete the clinical within 6 months. I understand that I have to book within 48 hours of payment and confirm future appointment days. I understand that, I will receive the service on the same day every month. I understand that in case of an emergency, I have 3 days to rebook to keep standing appointment or the appointment for this month will be skipped to guarantee clinical research information is properly entered. I understand the usage of my pictures and research results, will be public sharing. I understand I can opt out, to the sharing of my information, upon signing client disclosure form. I understand that after completion of payment, services will begin in the month of June.
