The primary contact is the legal guardian completing this registration on behalf of the player.

This Agreement is by and between Game Prep Baseball Academy, Inc. ("The Organization") and the Parent/Guardian of the Player participating in this Program managed or overseen by the Organization. The Organization is a private entity and includes its owners, directors and employees, affiliates, organizers, sponsors, representatives, agents and participants. In consideration of the details contained herein, the Parent/Guardian(s) acknowledge and agree to the following terms and conditions of this agreement.

I/We are the parent(s) and/or legal guardian(s) of the minor named as the Player and have the legal authority to execute this agreement.

I/We approve the Player's participation in any and all activities as established by the Organization and acknowledge and agree that such activities may result in serious injury and protective equipment may not prevent all injuries to the Player.

I/We agree to waive, release, indemnify and hold harmless Game Prep Baseball Academy, Inc., their owners, directors, employees, affiliates, organizers, sponsors, representatives, agents and participants from any and all claims, demands, liabilities, costs and expenses arising from injury to the Player, whether from negligence or for any cause.

CANCELLATION POLICY

All Cancellations Must Be In Writing and Received at info@gameprepbaseballacademy.com

Cancellation Deadline: November 4th.

Cancellations Received On or Before the Deadline Will Be Eligible for a Refund of 75% of The Payment Received.

Cancellations Received After the Deadline will Not Be Eligible for Any Refund of Payment(s) Received.

Payments Received will not be Discounted or Refunded in Part or In Full in the event the Player does not attend scheduled activities.

The electronic acceptance of this Agreement validates and executes all the Terms and Conditions detailed in this Agreement.

Game Prep Baseball Academy, Inc. Coaches, Staff, Instructors and personnel will follow the protocols set forth by the county of San Mateo and posted in the facility, however there is still a risk that someone could catch Covid-19 or infect others with Covid-19 involved in this program. If you choose to participate in our program, you must accept full risk and acknowledge GamePrep Coaches, Staff, Instructors, personnel and/or players and families involved in the program, are not responsible or liable in the event of a transmittance of Covid-19 to any participant.

The electronic acceptance of this Waiver validates and executes all the Terms and Conditions of this Waiver.
