Please note: There are TWO parts to the Scrip process

Part 1: Google Form- Fill out the following Google Form to indicate WHO you want your donations to go to.

Part 2: PAY (via PayPal or check) for the donations you intend to make in this Google Form. After completing the Google Form, go back to www.sverocks.com/scrip and click the yellow "Donate" button or use the link at the end of the form. Enter the total amount from this Google Form and PAY via PayPal.  If paying by check, please turn the check into the office by Thursday, April 18th.

**You should receive a confirmation email from PayPal once your donation is complete.**
